What is Plantar Fascitis and how does it cause Heel Pain? It is basically an overuse syndrome from working on concrete or over training that causes the long thick ligament on the bottom of the foot to become strained and inflamed. When the plantar fascia pulls away from the heel bone it creates a heel spur and causes heel pain.

ESWT, or Extracorporeal Shockwave Therapy is similar to Lithotripsy for kidney stones. Sound waves, or pressure waves go through water and are targeted by ultrasound guidance to break up the build up of scar tissue or plantar fibrosis at the attachment of the heel spur into the calcaneus, or heel bone.

Treatment for heel pain ESWT is provided right in the office and takes about a half hour. Regional anesthesia is provided by numbing the area and the heel is placed on the water filled cushion through which the sound waves go through. This gentle heel pain  treatment breaks up the scar tissue allowing “neovascularization ” to occur and heal the plantar fascia area correctly. New tissue then grows through the broken up scar tissue and over the next three months healing occurs to the treated area, not just putting a bandaid on it. Of course most patients can be successfully treated with conservative heel pain treatments like orthotics, corticosteroid injections and stretching. Shockwave Therapy is definitely worth considering before surgery.
